12th Annual CIPA EVVY AWARDS 2006 Entry Form

Colorado Independent Publishers Association (CIPA) and CIPA Education and Literacy Foundation (CIPA ELF) are proud to announce the call for entries for the Twelfth Annual Book Award competition for excellence in independent publishing, the EVVY Awards, named in honor of Evelyn Kaye, CIPA's founder.

All entries will be judged by three independent experts from a variety of venues. The books will be judged on various criteria including content, originality, cover design, book layout, writing quality, information, and interest.

Finalists in each category will be notified by early June 2006 and the winners will be announced at the Awards Dinner in June 2006. All members and guests are invited to attend this prestigious event as we recognize, honor and celebrate those who are setting the standards for all independent publishers.

Submission Instructions

1. By April 1, 2006 send four books and one flat book cover* to:

2. A fully completed entry form must be taped to the inside of EACH book. LEGACY Category entries must also choose a sub-category for judging. (Fiction, How To, Etc.) (print your form submission which opens in the next window once you've clicked on the submit button below.)

3. The entry fee is $75.00 per title per category entered. ($100.00 for Legacy category) Pay online and print entry form located below.

4. A copy of the Rules and list of Categories may be found on our Awards page or picked up at any CIPA meeting

5. Failure to comply with these requirements or the Rules may result in disqualification of the entry and forfeiture of the entry fee.

Category Note Explanations (Return to Category Selections)

(1) CHILDREN'S includes stories and picture books

(2) TRAVEL includes travel guides and travel essays

(3) DIET/HEALTH/FITNESS includes cookbooks

(4) LEGACY: Any independently published book that has not been entered in any prior EVVY competition AND has a publication date prior to 2005. Entry in this category is limited to one book per member. LEGACY CATEGORY ENTRY FEE IS $100

(5) All workbooks MUST be entered in the WORKBOOK category. They may also be entered in up to 2 other categories if desired. (see Rules)

(6) Entries will be accepted in the MISCELLANEOUS category only if no other category applies and the required written justification is approved by the Awards Committee. The entry fee for disapproved entries will not be refunded.
